We are hiring IMMEDIATELY for a Case Manager to join one of our community mental health providers!

Summary:  The Case Manager performs the duties and possesses the abilities as outlined below.

Job Summary:

  • Planning, developing, and implementing services for youth.
  • Collaborate with DCYF in delivering services to each youth.
  • Develops the ISTP (Individualized Service and Treatment Plan) and IBMP (Individualized Behavior Management Plan) for each youth utilizing the Child Family Team for input.
  • Meets with residents regularly to check in about goals and treatment.
  • Arranges for counseling as described in the ISTP.
  • Coordinates mental health, drug/alcohol, medical or other treatment as described in the ISTP.
  • Reviews and participates in the development of the youth’s Individual Education Plan (IEP) in coordination with the child/family team. This includes advocating with the local school district to ensure that the youth receives appropriate educational services.
  • Coordinates and ensures the inclusion of the child/family team in planning and decision-making processes.
  • Provides ongoing assessment of services and support needs of the client including advocating for youth to be moved to a less intensive support structure as their functioning improves.
  • Ensures that services provided are documented in the individual client file.
  • Attends all meetings relevant to each client’s case and provides relevant documentation to the client’s wraparound support team.
  • Informs staff of any changes, updates, or concerns weekly at staff meetings or arranges debriefing meetings as needed for urgent matters.
  • Facilitates and/ or supervises groups for skill development as needed.
  • Provides direct care coverage as needed.
  • Take part in developing and/ or coordinating in-service training for staff members as needed.
  • Take part in the development of therapeutic interventions and house meetings as needed.

  • Resolves conflicts using effective conflict resolution techniques;
  • Manages and works in a team environment that encourages individual initiative;
  • Communicates clearly and concisely, both orally and in writing;
  • Analyzes interpret and collect, a variety of information, to determine appropriate and reasonable courses of action;
  • Prepares clear and concise reports;
  • Works a flexible schedule that includes evenings and weekends, on-call and emergency situations;
  • Communicates and works with individuals with multiple, severe and chronic disabilities, mental illness, and diverse social and economic backgrounds;
  • Completes assessments and evaluate needs and strengths of referred residents, and provide appropriate information and referral services and follow-up;
  • Establishes and maintains effective working relationships with those contacted in the course of work including social workers, stakeholders, staff, the public, and residents
  • Allocates limited time and resources in a cost and result in an effective manner;
  • Operates office equipment including computers and supporting peripherals, as well as internet, research, database, word processing, and spreadsheet applications; and
  • Possesses aptitude and interests necessary to further develop knowledge and abilities.
